Position Overview
We are seeking a highly motivated, experienced Family Nurse Practitioner (FNP) to provide telehealth-based primary care services focused on closing gaps in care and improving patient outcomes. This role emphasizes proactive patient outreach, comprehensive virtual visits, and efficient management of multiple clinical priorities. The ideal candidate thrives in a fast-paced remote environment and is committed to delivering high-quality, patient-centered care within Medicaid and underserved populations.
Key Responsibilities
- Conduct comprehensive telehealth visits for primary care services across a multi-state Medicaid patient panel
- Identify and close gaps in care including preventive screenings, chronic disease management, and medication reconciliation
- Provide postpartum and neonatal follow-up care via virtual visits
- Screen and manage behavioral health conditions and substance use disorders (SUD) within clinical scope
- Execute proactive patient outreach to engage at-risk and lost-to-care patients
- Meet established productivity, outreach, and efficiency benchmarks
- Maintain accurate, timely clinical documentation in the EHR
- Coordinate care with interdisciplinary teams, specialists, and community health resources
- Ensure ongoing compliance with regulatory, quality, and payer requirements across all licensed states
Required Qualifications
- Active, unrestricted FNP license in at least one of the following states: Ohio (OH), Indiana (IN), Georgia (GA), or Maryland (MD)
- Board Certification as a Family Nurse Practitioner (FNP-BC or FNP-C)
- Minimum 2 years of clinical experience as an FNP
- Demonstrated experience delivering care via telehealth platforms
- Prior experience working with Medicaid or underserved populations
- Experience providing postpartum and/or neonatal care
- Proficiency with EHR systems and virtual care technology
- Strong clinical assessment, documentation, and independent decision-making skills
- Ability to manage high-volume outreach and multiple clinical priorities simultaneously
- HIPAA-compliant, dedicated home workspace suitable for telehealth visits
Preferred Qualifications
- Active FNP licensure in multiple states; OH, IN, GA, and/or MD (multi-state licensure strongly preferred)
- Behavioral health screening and treatment experience
- Experience treating patients with Substance Use Disorders (SUD)
- Background in value-based care models and quality improvement
- Bilingual; Spanish/English fluency a plus

- Contribute to quality measure improvement across the patient panel

Compensation & Benefits
Base Pay
This is a full-time, hourly non-exempt position with an annual equivalent compensation of $114,400, commensurate with experience, licensure, and state coverage.
Signing Bonus
Candidates who accept a contingency offer pending completion of credentialing and health plan affiliation with our clinic partners in one or more states (OH, IN, GA) are eligible for a one-time signing bonus of $3,000. The bonus is paid on the first day of patient-facing clinical work, once all required credentialing and affiliation steps are complete. Candidates who are fully credentialed and affiliated in all applicable states prior to offer acceptance are not eligible for the signing bonus. The signing bonus is considered taxable compensation and is subject to applicable withholdings.
